Position Summary

The Wound Care Nurse is responsible for assessing, treating, and monitoring a variety of acute and chronic wounds. This role requires clinical expertise, attention to detail, and a patient-centered approach to promote healing and prevent complications.

Key Responsibilities
  • Assess and evaluate wounds, including pressure ulcers, surgical wounds, and diabetic ulcers

  • Develop and implement individualized wound care treatment plans

  • Perform wound dressing changes and advanced wound care procedures

  • Monitor healing progress and adjust care plans as needed

  • Educate clients and caregivers on wound care management and prevention strategies

  • Collaborate with physicians and healthcare teams to optimize patient outcomes

  • Maintain accurate, detailed, and timely documentation

  • Ensure infection control standards and best practices are followed

  • Provide recommendations for pressure relief, nutrition, and skin integrity

Qualifications & Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) with a valid Ontario license

  • Certification or specialized training in wound care (preferred)

  • Minimum of 1–2 years of wound care experience

  • Strong knowledge of wound assessment and treatment techniques

  • Excellent clinical judgment and critical thinking skills

  • Strong communication and patient education skills

  • Ability to work independently in a home care setting

  • Valid driver’s license and reliable transportation (preferred)
